how to use with a Custom Membership Provider?

Since my hosting doesn't allow the databases to be accessed via "dbo" user, and since the Microsoft default provider has fixed into the code this user to connect with the database, I would like to use this library even with a custom Memebership Provider. is it possible?

If by this you mean you would like to be able to use database generation/database context intializer strategies (DropCreateDatabaseAlways, DropCreateIfModelChanges) then you will have to buy dedicated hosting, where you are in control of the sql server logins, and you can manage permissions to allow application to manage database in this way.

Otherwise you can still use this provider with a shared host, but you will need to keep database schema synchronization by hand. Easiest way is to use Entity Framework Migrations and use the generated .SQL scripts to upgrade database schemas. Either with SQL Server Management Studio if your hosting allows it, or online running the scripts.
